When you are SURE your book report is complete and correct, click on the "To Mr. Carey" button.

Add your signature after the form is received at school and printed. Hand in the printed form on the due date. That's all there is to it!

Check List...A Complete Book Report Has:

• The complete book TITLE, capital letters on important words. (10 points)
• Author's full name. Spelling! Capitals! (10 points)
• Book type. (5 points)
• Date turned in. (5 points)
• Minimum TEN-sentence summary about the book. (50 points)
• Evaluation sentence: Why you liked or didn't like your book. (10 points)
• Your signature. (10 points)
