A vaccination clinic for 7th and 12th grade students will be held on May 23 from 11:00 AM to 1:00 PM. Students must have a copy of their parent/guardian's ID and the back of the student's insurance card. The following documents must also be signed and returned to the school nurse by May 16:
- Consent and Financial Form
- Notice of Privacy Practices (pg. 4)
- Screening Checklist for Children & Teens
- Off Site Vaccine Clinic Patient Information
- Parent consent for 7th/12th grade school Immunization Clinic
